§ 5.40.060. Application shall contain specific information.

  • The application for a permit shall be submitted by the person who either owns or is responsible for managing and operating the carnival, and shall contain the following information:

    (1)

    The name and permanent address of the person making such application.

    (2)

    The names and permanent addresses of the owners of the carnival.

    (3)

    The name, sex, age, and permanent address of all employees, contractors, attendants, etc. who will be responsible for staffing carnival operations.

    (4)

    A list by name and description of all shows, rides, booths, and other attractions operated in connection with the carnival. For those rides requiring it, the applicant shall provide evidence that such rides have current amusement compliance stickers issued by the state department of insurance.

    (5)

    A description of the property where such proposed show is to be located.

    (6)

    The dates and times during which the carnival will operate.
